Bubs has been on 9mg of losec since 4 weeks old (almost 8 weeks old now) and 500grams heavier than before -4.7kg he has been unsettled again after and during feeds so figure its the reflux, rang the pediatrician and he said to up it to 10mg a hole capsule how long does it take to see an improvement with uping the dose?

It depends on the severity of any oesphagitis etc. You should start seeing improvements within a few days, but if bubs has a lot of acid burns in his throat then it could take a couple of weeks for that to heal.

It can take up to three weeks to see the full effects but that isn't much of an increase in dose. 10mg is however a good dose for his weight and there is still room to move if the paediatrician decides it is needed. I would give it a week and then let the paediatrician know or try some other approaches if it isn't working enough. By other approaches I mean diet or natural therapies such as probiotics or slippery elm.
Losec isn't a very weight dependant medication so if this is an increase in symptoms I would probably be looking for another cause. Did you know that 40% of our babies with reflux have a cow's milk allergy? Also, are you giving the granules correctly? That is not with milk. He has some good feeds a few after the losec and then its down hill he just screams and screams and screams!

I have been givign the granules at least half an hour before food and lately iwith 0.5ml of pamol as the pediatrican said I could wash it down with 1ml of pamol with no effect - but will switch back to using water as dont like the thought of using drugs! just did that as he was having pamol for 48 hours after his injections the other week.

I did try a probiotic formula but hes use to the thick reflux ones so it was a bit fast flowing for him even with the water/breastmilk teat, where can you get probiotics from and do you add them to the formula?

he has been a little mucusy in his nose wonder if that is a cows milk allergy? will mention it to the pediatrician when I ring him this week \:\) thanks I hope he improves soon too!

Formulas don't contain probiotics but some contain prebiotics. You can get probiotics at pharmacies and health food stores, and online. http://www.healthpost.co.nz only charge $3 courier and their prices are reasonable - probably cheaper than going out and buying it, and usually delivered the day after ordering. Let me know if you want me to hunt one out there that would be suitable. Kyodophilus is available there from memory and it is dairy, gluten free and has a child dosage. Half a capsule and use it can be added to formula.
The thickened formula may be a problem in itself as it can sit in the throat for a while if refluxed. The mucous in his nose could be reflux or an allergy.
